The Only Surgery in History With a 300% Mortality Rate

Robert Liston was the fastest surgeon in 19th-century Britain. In one legendary operation, he amputated a leg in under three minutes — and killed three people in the process.

The Short Answer: Robert Liston's famous leg amputation achieved a 300% mortality rate: the patient died of gangrene, an assistant lost fingers and died of gangrene, and a spectator died of fright. It remains the only operation in history to kill three people at once.

One operation. Three deaths. The patient, the surgical assistant, and a spectator. It remains the most lethally efficient procedure in recorded surgical history, and it happened because speed was the only anaesthetic available.

The Fastest Knife in the West End

Robert Liston (1794-1847) was the most famous surgeon in London and, by all accounts, the fastest. In an era before anaesthesia, speed was not a stylistic preference. It was a medical necessity. The longer an operation lasted, the greater the blood loss, the deeper the shock, and the higher the chance the patient would die on the table.

Liston could amputate a leg in under 30 seconds. He was so proud of his speed that before each operation, he would turn to the gallery of medical students and spectators packed into the surgical theatre and call out: "Time me, gentlemen, time me!"

He was tall, physically powerful, and operated with an intensity that bordered on theatrical. Patients were held down by assistants. There was no other option. Liston worked with terrifying speed and, for his era, genuine brilliance.

The Infamous Operation

But brilliance and haste make dangerous companions. In his most notorious case, Liston amputated a patient's leg in approximately two and a half minutes. In his furious sawing, he also slashed through his surgical assistant's fingers. The assistant subsequently developed gangrene and died.

During the same frenzied procedure, Liston's knife reportedly sliced through the coat tails of a spectator standing too close. The spectator, believing he had been stabbed, collapsed and died of shock on the spot.

The patient, the intended subject of the operation, also died, likely from post-surgical infection.

Three deaths from a single procedure. A 300% mortality rate. No other operation in recorded medical history has matched it.

The Twist

The same Robert Liston went on to perform the first surgery in Europe under modern ether anaesthesia, on 21 December 1846 at University College Hospital in London. His reaction afterwards was characteristically blunt: "This Yankee dodge beats mesmerism hollow!"

In that single moment, speed became irrelevant. The frantic, blood-soaked race against consciousness was over. The era of careful, methodical, precise surgery had begun.

From Speed to Precision

Surgery used to be defined by speed. Today it is defined by precision, teamwork, communication, and professionalism. The journey from Liston's blood-soaked frock coat to the modern sterile operating theatre is one of the greatest transformations in human history. It started because someone decided that doing things properly mattered more than doing them fast.
