Naked Surgeons Shed FEWER Bacteria Than Clothed Ones — Here's Why That Matters

What if surgeons would actually shed fewer bacteria without any clothes on? It sounds absurd, but the science is clear — and the real lesson is not what you think.

The Study That Rewrote the Rules on Surgical Attire

A landmark review published in Clinical Infectious Diseases (Bartek, Verdial & Dellinger, 2017) pulled together decades of dispersal research and confirmed something counterintuitive: naked men shed roughly one-third to one-half the bacteria of the same men wearing scrubs or street clothes.

The original data goes back to Hill et al. in The Lancet (1974), where researchers measured airborne bacteria around subjects in various states of dress. The clothed subjects consistently produced more colony-forming units. Why? Fabric friction.

Your Clothes Are Launching Bacteria Into the Air

Every time clothing rubs against skin, it acts like a catapult for skin cells. Those cells — called squames — carry bacteria on their surface. The friction does not just dislodge them; it flings them upward and outward into the surrounding air.

The study also confirmed that men shed approximately twice as many bacteria as women, and that the primary dispersal site is the perineal area — not the head or arms. So the biggest contamination source in the operating room is not what most people assume.

Here is what makes this clinically relevant: the type of fabric, the weave density, and how the garment fits against the body all determine how aggressively it launches particles. A loose, rough-textured scrub top creates far more dispersal than a smooth, fitted garment made from tightly woven material.
